Dr. Adam Cohen and Dr. Taylor DeLaura welcome referrals from general dentists, periodontists, orthodontists, endodontists, and other healthcare providers for the full range of oral and maxillofacial surgical services our practice offers; including dental implant placement and restoration, wisdom tooth extractions, bone grafting, sinus lifts, tooth exposures, apicoectomies, oral pathology evaluation and biopsy, TMJ surgery, facial trauma surgery, and office-based anesthesia for complex surgical cases.
How To Refer Your Patient
To refer your patient to NorthStar Oral Surgery, please complete the referral form provided below. The form includes fields for the patient’s contact information, diagnosis details, the specific procedure or evaluation being requested, and any relevant clinical notes or imaging that will help our surgical team prepare for the consultation. Once completed, the form may be submitted directly to our office or provided to the patient to bring with them when scheduling their appointment.
